2. Templates
To submit a safety assessment template:
- select the work practice(s) from the list below for which you are applying for consent
- download a:
- new consent template if you do not have consent for the work practice
- continuing consent template if you are submitting a safety assessment for a work practice that that you are carrying out that has previously been granted consent
- complete the template and upload it to HSE when requested during the application process – start or continue your application
You can download each template as either a Microsoft Word document or an OpenOffice document.

The deliberate administration of radioactive substances to people or animals for medical or veterinary diagnosis, treatment or research
The deliberate addition of radioactive substances in the production or manufacture of consumer products or other products, including medicinal products
Operation of an accelerator
Except
- when operated as part industrial radiography or
- an electron microscope
Industrial radiography
Industrial irradiation
Working with a high-activity sealed source (HASS)
Working on any facility for the long-term storage of radioactive waste or disposal of radioactive waste
This includes facilities managing radioactive waste for this purpose but not any such facility situated on a site licensed under section 1 of the Nuclear Installations Act 1965
