Specified Animal Pathogens forms
This page provides the forms to assist you in applying for a new licence to work with or possess a specified animal pathogen.
A SAPO1 must be completed to apply for a new licence, a licence renewal or to notify a significant change to an existing SAPO licence. Please ensure you include your risk assessment with the SAPO1 form.
A SAPO2 form is for notifying the licensing authority of an accident or dangerous occurrence relating to loss of containment of a specified animal pathogen.

Downloadable forms
These forms may be completed and sent to the SAPO Applications Officer [email protected]. They may also be printed, signed and posted to HSE although an electronic version is preferred.
