This OM details the revised charges for statutory medical examinations and describes which forms should be used. It replaces OM 2004/07 which should now be cancelled.

Purpose

Revised charges

The Health and Safety and Nuclear (Fees) Regulations came into force on 6 April 2016.
Schedules 5 and 6 of the Regulations list the revised charges for statutory medical examinations carried out by Medical Inspectors [MEDIs] formerly known as EMAs and are reproduced at Appendix 1 for reference.

Forms

MEDIs are required to complete and forward an A/cs 4 [rev 08.08] Request for Invoice form to PEFD. This is available on the Intranet.

A/cs4 value added tax [VAT] entries

When completing the A/cs 4. Note that it contains a VAT analysis field which requires requesting officers to state the VAT rate charged for the service. In the case of medical tests performed as a result of statutory legislation, they are outside the scope of VAT and therefore code 06 must be quoted on the form. Further information on the completion of this form can be found in HSE Finance Code, Section H.
