A Volunteer Consent Form should be completed for each person willing to take part in your study. It should be issued in conjunction with the Subject Information Sheet discussed under ‘Submit a Proposal’. The consent form must first be signed off by the Committee Chair, and this will be arranged by the Secretariat once you have obtained the Committee’s ethical approval for the study.
For your convenience, a model form covering the issues for which you may need to obtain volunteer consent can be downloaded by clicking on the link below. You may not need to include all the elements in the model consent form, in which case you should remove the inappropriate items.
