"5 The information should be copied from the outside worker’s health record and updated whenever a medical examination or review takes place. It is not necessary for an Appointed Doctor to make these entries in the passbook. The date relates to when the health record was signed by an appointed doctor and not the dates of any recorded clinical tests or examinations. Where an outside worker has more than one concurrent employer, they may like to consider numbering themselves 1, 2 and show that number against the date column." "6 The information should be copied from the outside worker’s health record and updated whenever a medical examination or review takes place. Where an outside worker has more than one concurrent employer, they may like to consider numbering themselves 1, 2 and show that number against the date column.
Although Appointed Doctors may carry out a routine medical review one month before or after the end of each 12-month period (the anniversary review date), for continuous classification, the review date must always be treated as that anniversary date every year. For example, if the anniversary review date is 10 July and the review is carried out between 10 June and 10 August, the next review will be due on 10 July the following year."

Eexternal = Effective dose from external radiation
Einternal = 50-year committed effective dose from internal radiation

"7 If an outside worker has more than one current employer, these employers should preferably agree a ‘lead’ employer for dose assessment purposes. It would also be desirable for them to nominate one ADS to co-ordinate dose record-keeping."   "8 Separate entries are needed where assessments are made of doses from external radiation and internal radiation (recognising that assessment periods may not be the same). Separate entries are also needed for assessments of non-uniform exposure, eg for dose to the eye lens or skin of the hand if these are separately assessed. See Section 7 for information on annual total, where the 5-year limit applies." 9 Separate entries are needed where assessments are made of doses from external radiation and internal radiation (recognising that assessment periods may not be the same).
