Description Failed to ensure that a safe system of work including the provision of appropriate information, instruction and training was provided with respect to the handling and use of gas cylinders, in that employee suffered serious personal injury whilst attempting to change a compressed gas cylinder charged at 220 barg when there was an instantaneous release of the contents of the gas cylinder resulting in the cylinder acting as a projectile and striking him.
