Description Apache Beryl B Improvement Notice 309102712 - Apache is significantly failing to meet health & safety arrangements as detailed in the production technicians rota. In November 2017, it was established that as part of the change in rota pattern, there should be 4 production technicians each, on day and night shifts, for routine platform operations including an allowance for 2 control room operators (CROs) per shift, but with an allowed allocation of 5 per shift to cover sickness / absence / rota breaks as reflected in the current rota. The inspection found that both day and night shifts only had 3 production technicians, including 1 CRO, which is significantly lower than that originally identified by you as what was required to safely operate the plant, conduct routine operations and allow personnel to have sufficient rest periods / breaks. Whilst there was some cover using trainees / apprentices / assistants, they are not deemed to be fully competent as per Apache's own competency management system and cannot be considered to provide adequate cover. This reduced production technicians manning is not an isolated incident and has been ongoing since early 2018. Adequate effect has therefore not been given to health and safety arrangements as reduced production technicians manning has the potential to increase fatigue and a decreased capacity to carry out critical work scopes which affect health and safety.
