Description Beryl B - 1* IN 308604250 served as the duty holder not following the procedures and arrangements described in its current safety case which may affect the health and safety of persons. The current version of accepted safety case ANS-BB-TS-001, in Part 3 Section 2.2 page 3-10, requires that documents on the Business Management System (BMS) are reviewed at periodic intervals and that validity of documents are checked regularly. This has not been done. Several important procedure documents available on the BMS and used for undertaking tasks that affect health and safety of persons had not been reviewed or checked for validity at their recommended intervals, including in particular: 1. ANS-TI-028 which was due for review every 12 months but had not been reviewed since 30/10/2006; 2. ANS-SOP-043 for which no review was undertaken on its 30/11/2016 review date; 3. ANS-MTC-003 for which no review was undertaken on its January 2016 review date; 4. ANS-FX0OMA-002 which was due for review every 12 months but had not been reviewed since December 2015; and 5. ANS-SSW-001 for which no review was undertaken on its Dec 2016 review date.
