Description I carried out an inspection of the Clyde installation between the 4th and the 7th of November 2014. During my inspection I sought evidence of the effective implementation of your maintenance management system. To this end, I requested details of the status of safety critical work orders together with copies of maintenance procedures ‘P6 Request for Extension to Inspection and Repair Order Due Date’ (procedure number MNT-PRO-TLM-004) and ‘P3 Work Deferral and PM/JP Amendment’ (MNT-PRO-TLM-006), which together set out the process for approving deferral of your safety critical work orders. I was provided with records that showed that as of the 6th November 2014, there were 182 outstanding safety critical work orders, 159 of which had not been approved for deferral. Upon receipt of this information and with regard to the deferral processes described in your procedures, I formed the opinion that you had failed to properly implement your procedures. Consequently, in my opinion, you had failed to give effect to such arrangements as are appropriate for the effective control of your preventative and protective measures.
