The Health and Safety Executive is investigating an incident which took place at the Billingham based Terra Nitrogen (UK) Limited site in the early hours of Tuesday, 1 June. Inspectors from HSE have now carried out a preliminary investigation on what is a joint investigation with the Environment Agency.

HSE's preliminary investigation involved establishing the facts of what happened during the incident and ensuring the site was made safe by the operator.

Inspectors now have access to plant and equipment involved in the incident, some of which has been removed for more detailed examination. The aim is to establish why the incident occurred looking at management and operating systems including an examination of plant equipment, and interviews with witnesses. The investigation is drawing support from chemical, mechanical, control and instrumentation specialists as necessary.

It is too early to say when the root causes of the incident will be known.

A more detailed investigation will follow to establish why the incident occurred looking at management and operating systems including a scientific examination of plant equipment and interviews with any witnesses. The investigation will draw support from chemical, mechanical , control and instrumentation specialists as necessary.'
