Approval of Classification - application details

  This is a new (November 2007) form that gives greater help to applicants on the required information.  The form is a Microsoft Excel spreadsheet . Please download a new form for each application to ensure that the formulae in the spreadsheet are not lost.  Please do not use previous versions of the spreadsheet. To print the contents of the table in Part B highlight the table headings and entries only and print the "selected range" in landscape format.
