What is this form for?
Many costs can arise from an incident. This form helps you to work out what they are for your
organisation. It lists what the costs can include, but there may be others as well.
The form is designed to be relevant for as many incidents as possible.
Not every cost listed will apply for every incident. Ignore those you do
not believe to have arisen. But be careful you don't exclude any relevant costs. If
you do you will underestimate the true costs of the incident.

What can I do with the form?

How to complete the form
For each cost you need to consider whether it is relevant. If it
is not relevant, ignore it.
If it is relevant, you can do one of two things:
- you can enter the total amount for this cost directly onto the form; or
- if you do not know the total cost, but you do know how much time was involved you
can record this instead.
Some of the times should be recorded in hours, while others should be in days. The form indicates
which to use.
If you record the time spent, you also need to record the hourly or daily rate for the cost.
Some of the costs allow you to use an average value from a reference table for these, otherwise
you have to input your own.
The costs which have reference data are shown by
on the right hand side of the costs column. Click on the

As you enter your information, the form will automatically be updated. You will see this on screen.
After you have entered all your information, you can preview the report before printing it. To do this,
click on the ‘print preview’ button at the bottom of the form.

Online help when completing the form
The online form has some built in information to assist you to complete it.
For helpful information for a costing, click the
icon where available and a help box will pop up. To close the help box either:
- click elsewhere on the form;
- click the close link on the help box; or
- press the Escape key.
Some of the costs have indicative values to assist you. These are shown by
where available. Clicking on this opens
a pop up data box with a list of possible values and descriptions. The options available
to you are:
- click on a value and this will be automatically entered into the relevant input box;
- click the close link on the help box; or
- press the Escape key.
All three options will close the data box.

What do I need to do before completing the form?
At present you cannot save the form online. You need to complete
it in one go or print out the part completed version. However, if you do this, you
will need to re-enter the data next time. You should give yourself sufficient time to
complete the form and we suggest you have your costs to hand.
We think it should take you about 10-15 minutes to complete the form.

Handy Hints
If this is the first time you have used the form, we suggest you look at it
(and the online help) before completing it. You will then know what information you
need to provide.
Also, you may wish to look at and make note
of the reference data that you will use. This will save you time later on.
You may also wish to write down the costs you already know to
save you time looking for them.
It is useful, where you can, to record the costs as soon as possible after an
incident happens and while you are investigating what went wrong. You may want to use
a blank copy of the form to assist you.
For each type of cost, consider all the costs involving:
- people;
- premises;
- plant, equipment and substances; and
- procedures.
That way you will ensure you don't leave anything out.

accurate data on them. We would also like to display copies of completed forms on this
website to share the information. This will only happen with your cooperation.
Any material used will be shown anonymously, unless you tell that you wish to be named.
All information sent to us will be held in the strictest
confidence and only used for updating this website. HSE will not
use the information for any other purpose, nor disclose it to any third party.

some contact details to check the case study is genuine. This information will not be
