By organisation we mean a situation which has had an organisation wide impact eg:

the introduction of an organisation wide training program; or
the costs of annual losses to your whole organisation.

Please describe the circumstances leading up to the incident in as much detail as possible.

Please describe the consequences of the incident both to the individual and your organisation.
Include any knock on effects such as production delays, missed orders, loss of business, compensation claims etc.

Breakdown of costs

Please give details on what you calculated the costs of the incident to be.
Please provide a cost breakdown if possible. If you are able to complete the Incident Cost Calculator on this website we would be very grateful. But you will need to do this first.

Please give background information on your organisation to give some context to people reading your case study. This should include for example:

the nature of your business,
the sorts of activities you undertake,
where you operate etc.

Description of costings methodology

Please give details of the costings methodology you adopted for your study. Please include information on:

the type of study you ran ie

a real time study;
a retrospective study; or
a combination of these two methods.

the duration of the study;
how many recorded events occurred during that time;
the type of events which occurred;
the definition of a qualifying event (accident) you used;
what events you excluded from the study and why;
whether your study was restricted in any other way eg geographical location etc

Please describe in as much detail as possible the changes made to address the issue.
Where possible, please include the costs of implementing these measures.

Business benefits and cost savings

Please describe in as much detail as possible the benefits realised following the changes eg money saved, efficiency increased etc.
Where possible, please quantify these benefits in financial terms.
If you expect the investment costs to be repaid over a period of time, or year on year benefits to accrue, please include details.
Please also include any ongoing maintenance costs for the programs or measures introduced.

Costs to organisation

Please detail the costs your organisation was incurring before the change(s) made. You should say how much was involved and over what period eg £30000 a month.
Where you can, please provide a breakdown of these costs eg wages for absent workers, lost production etc. You may find the cost areas on the Incident Cost Calculator helpful.
It will also be informative if you can put these costs into perspective eg

% of annual turnover;
% of sales revenue;
% of wage bill etc

What the organisation did

Please describe in as much detail as you can the changes you made.
Please include the costs of implementing the changes, breaking them down where possible into their component parts.
