Step 4.1

If the inspection is now completed:

  • briefly consider whether the inspection objectives were achieved
  • where necessary prepare a report recording the outcome and the reasons for the decisions taken
  • complete any inspection rating(s) required

Step 4.2

Are any documents to be dispatched


Prepare and despatch, as appropriate, any required letters, reports or other documents to:

  • the dutyholder
  • any employee representatives
  • other parts of HSE
  • any other enforcing authority
  • any other interested parties

Send all such items to external recipients:

  • within 10 working days following the inspection, or
  • inform the recipient(s) when they will be received, if despatch after this date is likely

Step 4.3

For: Inspector/Team leader

Is further action needed?


Consider and take further action, as appropriate, to deal with issues arising at the inspection; this may involve:

  • revisiting the dutyholder at some point to verify that important controls remain in place over time
  • revisiting if proposed future changes to plant, processes or working arrangements are likely to have a significant impact on the overall health and safety assessment
  • checking that particular deficiencies, not the subject of notice procedures, have received attention
  • obtaining health and safety information on certain chemicals or substances found in use during the visit
  • arranging for HSE specialist staff to visit to give detailed advice on specific health and safety issues, including in support of potential enforcement action
  • arranging for other HSE staff to visit suppliers of products or substances found in use at the visit under circumstances which had s6 implications
  • raising with other agencies issues on the boundary of, or even outside, HSE's jurisdiction, such as certain aspects of fire safety or environmental risk
